供体集中维护模式在器官捐献中的应用效果研究

Abstract:

Objective To investigate the application effects of the centralized donor maintenance model inorgan donation. Methods This study collected information on potential organ donors maintained in our hospital's Organ Procurement Organization (OPO) from August 1,2023, to March 31,2024. The subjects were grouped according to the maintenance model: the control group used a decentralized maintenance model Except Donor Intensive Care Unit(DICU),while the experimental group used a centralized maintenance model (in the DICU). The effectiveness was evaluated using indicators such as organ yield, organ utilization rate, donor maintenance time, cold ischemia time of transplanted organs, and postoperative complication rates. Results The overall organ yield and utilization rates in the experimental group were higher than those in the control group, but the differences were not statistically significant (P 0.067,0.204, respectively). The donor maintenance time in the experimental group was longer than in the control group (P 0.001), and the cold ischemia time for donated kidneys and livers were shorter in the experimental group than in the control group (P 0.001). There were no statistically significant differences in the incidence of common postoperative complications between the two groups (P 0.05). Conclusion The centralized donor maintenance model can prolong donor maintenance time, shorten the cold ischemia time of kidneys and livers, and is expected to improve the quality of donated organs. It is worth promoting and applying.

摘要:

目的 探讨供体集中维护模式在器官捐献中的应用效果。方法 本研究收集 2023 年 8 月 1 日至 2024 年 3 月 31 日在广西医科大学第二附属医院器官获取组织中维护的潜在器官捐献者信息,按维护模式分组,对照组采用分散维护模式〔(donor intensive care unit,DICU)外〕,实验组采用集中维护模式(DICU),进行同期对照研究。评价指标包括器官产出率、器官利用率、供体维护时间、移植器官冷缺血时间及术后并发症发生率。结果 实验组各器官总体产出率和利用率均高于对照组,差异无统计学意义(P 分别为 0.067、0.204);实验组潜在器官捐献者维护时间长于对照组(P < 0.001);捐献肾脏及肝脏冷缺血时间实验组均短于对照组(P < 0.001);两组器官移植术后常见并发症发生率差异无统计学意义(P > 0.05)。结论 供体集中维护模式可延长捐献者维护时间、缩短肝肾冷缺血时间,对改善器官质量具有积极意义,值得推广应用。
