早期心输出量与儿童移植肝血流及肝功能相关性分析

Abstract:

Objective To analyze the correlation between cardiac output(CO) parameters and hepaticblood flow, liver function, and inflammatory response after pediatric liver transplantation. Methods Thisstudy included 39 pediatric liver transplant patients from Tianjin First Central Hospital. Cardiac output and circulatorydynamics parameters were collected on the first postoperative day using a non-invasive ultrasound cardiac output monitor. Liver function, hepatic blood flow, NT-proBNP, and inflammatory markers were recorded on the first, third, and fifthpostoperative days. Spearman correlation analysis was used to explore the relationship between cardiac output parameters and liver function, hepatic hemodynamics, cardiac function, and inflammation. Results CO was positively correlatedwith HA PSV and mean arterial pressure (MAP) on postoperative day 1(P 0.05) and negatively correlated with PVflow, aspartate transaminase (AST), white blood cell(WBC)(P 0.05), as well as NT-proBNP on postoperative day 5 (P 0.05). Cardiac index(CI) was negatively correlated with TBil on postoperative days 3 to 5(P 0.05). SVR waspositively correlated with HA PSV and AST on postoperative day 1, and with NT-proBNP on postoperative day 5(P0.05). CVP was negatively correlated with PV flow (P 0.05). Conclusion Early cardiac output following pediatric livertransplantation correlates with hepatic blood flow, liver function, NT-proBNP, and inflammatory markers, confirming its potential significance as an early risk indicator. This suggests early cardiac output may serve as a reference indicator for earlyassessment of transplanted liver function and hemodynamic risk.

摘要:

目的 分析儿童肝移植术后心输出量(cardiac output, CO)与肝血流、肝功能及炎症反应的相关性。方法 本研究纳入天津市第一中心医院 39 例儿童肝移植患者,于术后第 1 天通过无创超声心排量监测仪采集心输出量及循环动力学参数,并记录术后第 1、3、5 天的肝功能、肝血流、NT-proBNP 及炎症指标,采用 Spearman 相关分析探讨 CO 与肝功能、肝血流动力学、心功能及炎症之间的关系。结果 CO 与术后第1 天 HA PSV、MAP 呈正相关(P 0.05),且与门静脉流速(portal venous flow, PV flow)天冬氨酸转氨酶(aspartate transaminase,AST)、白细胞(white blood cell,WBC)呈负相关(P 0.05),与术后第 5 天 NT-proBNP 呈负相关(P 0.05),心指数(cardiac index, CI)与术后第 3 ~ 5 天总胆红素(total bilirubin,TBil)呈负相关(P 0.05)。SVR 与术后第 1 天肝动脉收缩期峰值流速(hepatic artery peak systolic velocity , HA PSV)、AST,术后第 5 天 NT-proBNP 呈正相关(P 0.05),CVP PV flow 呈负相关(P 0.05)。结论 儿童肝移植术后早期心输出量与肝血流、肝功能、NT-proBNP 及炎症指标之间存在相关性,证实其作为早期风险提示指标的潜在意义,提示可作为早期移植肝功能及血流风险参考指标。
